Dr. Yeager will lead a panel discussion among two of The IC's agency directors and a board member to describe "Hospital-2-Home," a successful collaboration between The IC and UC Health Memorial Hospital in Colorado Springs to facilitate successful transitions to home for patients with a complex or difficult discharge. Through H2H, The IC provides service assessment, coordination, and a network of local community-based organizations to provide support services that address social determinants of health for patients discharging from the hospital so they could successfully thrive at home. The H2H program worked with complex and potentially extended length-of-stay hospital patients and met their needs at home. Many patients who participated in the H2H program had Medicaid, Medicare, or no insurance at all. This interactive presentation will allow ample time for Q&A and will include detail of the financial gains and patient outcome and satisfaction improvements realized by both partners.
